Azwar was born in Langsa, May 3, 1966. Graduated from the Faculty of Medicine University of Syiah Kuala University in 1994, In 2001 he complete his study in the Professional Program of Microbiology In Airlangga University, In the year 2000 until now, was appointed as a lecture at the Faculty of Medice Syiah Kuala University. In 2016 he completing his doctoral degree at Faculty of Medice Gadjad Mada University. Now he get position as Head of Department Ent Zainoel Abidin Hospital/ Faculty of Medice, Syiah Kuala University.
